Unlike service pack two there aren't many functional changes, it's mainly just a big rollup so you don't have to make 50 runs at installing updates.
Anyone who has installed a fresh copy of Windows XP recently can attest to the fact that it became a nightmare.
If you update often this is less important, if it's been awhile then this is a must do.
